We participate as an in-network provider for the following plans:

• United Concordia NFFS plans
• Delta Premier plans
• Aetna PPO

We are pleased to assist all patients with ANY PPO insurance by submitting claims and accepting payment on their behalf. We are NOT able to receive benefits from HMO or DMO insurance plans. Estimated copayments will be collected at time of service for all plans, as dictated by the plan. Any residual copayment due after insurance payment will be billed promptly for payment by the account holder.

Although we do our very best to be informed about insurance coverage, please be aware that there are literally thousands of plans and it is impossible to know the details of every employer’s individual plan. Your insurance is a contract between you, your employer, and the insurance company. As an outside party, our ability to secure details can be limited. Please participate in your coverage so that we may work together to help you receive your best benefit.
